Family Doctor and Family Nurse Appointment
You must book an appointment to see the family doctor or the family nurse.
You can register for an appointment via the perearst24 online platform, by phone at 46 22 555, or in person at the center.
When booking, please state the reason (e.g., acute illness, need for a certificate, etc.).
The nurse assigns the appointment date and time based on the issue: the more urgent the health concern, the sooner you will be seen.
For acute illnesses, the initial examination—and lab tests if needed—are performed by the family nurse, who has specialized training. The nurse can always consult a physician.
Each visit is scheduled for 30 minutes. If multiple family members wish to see the doctor, please book separate times for each person.
Joining the Family Doctor’s Patient List
If you wish to become a patient of the Päevasaare Family Medical Center, please complete the Application to Register with a Family Doctor’s Patient List (download here: APPLICATION) and send it with a digital signature to avaldused@paevasaare.ee, or mail a paper copy to the center at Rahu 2/1, Kärdla, 92414 Hiiu County. You can also complete the application on site at the center.
You will be added to the patient list from the first day of the next calendar month, but family doctor services are available from the day you submit the application.
All newborns are automatically registered with their mother’s family doctor.
Prescriptions
Repeat prescriptions can be renewed in perearst24 or by phone. Requests are confirmed within 1–2 business days. Patients with chronic conditions should keep at least a two-week supply of their medications at home.
If you forgot to renew essential medications, you can also do this on weekends by calling the family doctor advice line at 1220.
Sick Leave Certificate (Certificate of Incapacity for Work)
To open a sick leave certificate, contact us on the first day of illness. Outside the center’s working hours, please send a notification via perearst24.
According to the Ministry of Social Affairs regulation (https://www.riigiteataja.ee/akt/107052024008?leiaKehtiv#para5), a sick leave certificate may be opened retroactively only in two cases:
1. if you have another document confirming the illness: an ambulance report, hospital discharge, or ER summary. In this case, the certificate may be opened within 2 days after falling ill;
2. if you fall ill—or a need for care/nursing arises—on a weekend or a national/public holiday, and you inform a healthcare professional by remote means or attend an appointment on the next business day.
To close a sick leave certificate, contact us at the agreed time by phone or via perearst24.
Driver’s Medical Certificate
Before applying, you must complete the Health Declaration in the patient portal: https://www.terviseportaal.ee. After that, book an appointment with the family nurse within one month. Bring your glasses or contact lenses if you use them for driving. Health requirements for drivers are set by a Government of the Republic regulation. The certificate is fee-based. When an e-certificate is issued, it is automatically sent to the Traffic Register.
Other Certificates
The family doctor’s center issues only those medical certificates that are required under Estonian laws and regulations.
Certificates requested by various institutions/organizations for events, camps, competitions, etc. are often not justified. In most cases, they can be replaced with a health declaration, in which the person (or a parent/guardian) takes responsibility for providing their health information. In Estonia, there is generally no single national portal or standard form for a camp/event health declaration. In practice, the organizer provides the form (or requests a free-form confirmation), because this is not a state-required medical certificate but rather the participant’s/parent’s confirmation of responsibility.
A medical certificate is a clinical assessment of a specific person’s health status and can be issued only when there are clear criteria on which to base that assessment.
Kaitseliit (Estonian Defence League): if, at the time of joining, a person holds a valid driver’s license, a separate medical examination is generally not required; otherwise, the medical examination is performed based on the health requirements for motor vehicle drivers.
Issuing certificates may be subject to a fee according to the price list. Payment does not guarantee that a medical certificate will be issued (the decision depends on the health assessment). Päevasaare’s price list is available on the services page.
